Below are the Relief Benefit Forms for both Local 80 and Southern Benefits.  If you are a member in good standing and are out of work due to an accident or illness, either at work or outside of it, you qualify for the Relief Benefit.  They Local benefit pays $40 per week up to 26 weeks.  Southern Benefits pays $300 a week and also covers your health fund for 34 hours per week for up to 26 weeks.

Simply print these out, have your attenting physician complete the required forms and retun them to the Local and Southern Benefits, respectively.  

It is your responsibility to notify the funds when you return to work.  Failure to do so could lead to forfeiture of future Relief Benefit claims.
